We work closely with local authorities, health providers, community organisations and voluntary sector partners to enable us to maximise resources, share expertise, and co-design services that are responsive to the needs of our communities. Health and Housing Project
The Health and Housing Project, the partnership between PCH and Livewell Southwest, was launched in 2021 to improve health and wellbeing outcomes for residents. By enabling them to live independently, safely, and healthily at home for longer, the project supports both individual wellbeing and the sustainability of our local health services.

2025-2028 Aims
The next phase of the project will focus on:
- Enhancing health outcomes
- Reducing healthcare demand
- Improving quality of life
- Delivering coordinated, accessible, and preventative services.
Four key themes of the project are:
- Admission Avoidance & Prevention
- Health Improvement & Early Intervention
- Integrated Place-Based Partnerships
- Digital Transformation, Data & PHM.
Key projects delivered to date
Trusted Assessor
Enhancing home safety and independence at home through timely home assessments, enabling access to aids and minor adaptions and reducing demand on services.
Wellbeing Officer
Providing tailored health and wellbeing support, helping residents to access services, reduce social isolation and to work towards their health and wellbeing goals.
Community Wellbeing Champion Programme
Empowering residents to support one another through a jointly developed programme. Champions receive training, a certification, and ongoing support to through a wellbeing champion network
Plymouth Living Lab
Exploring the use of digital health technologies in residents’ homes. This initiative connects residents, research and innovation, to pilot, co-design and evaluate new digital tools.
Digital Inclusion Scheme
Tackling digital exclusion by providing access to devices, connectivity and support. Helping residents to access online services, stay connected and build digital confidence.

Tracy Lowings, PCH resident who completed the Community Wellbeing Champion programme, said:
“The course was enlightening, and it was great to be amongst a group of such diverse individuals who all had a lot of different views. I now have more knowledge about wellbeing, where to find help and how to help my community. I would recommend it. Just two days, but a lifetime of information.”
A PCH resident who received support as part of the Digital Inclusion Scheme, said:
“I had Matt help me with my phone. I had a lot of pop ups coming up and didn't know where it was coming from.. Matt went through my phone and found the issue.
“What stood out to me was his patience with me as I know nothing about mobile phones, and he explained every avenue he was going down. He asked my permission to go into certain apps to remedy this. He was very courteous and treated me with respect, not like an old 'fuddy duddy', and made me feel comfortable in my own home.
“He looked after me and reassured me that what he had done should remedy the situation, but if I had any other problems he would come to see me. Nothing was too small or too big, which is good as I've said to Matt not everyone has someone to help, and this day and age we rely more and more on our phones. Things change so quick. I'm definitely not a techno person but I made notes of things to help me.
“I believe Matt is an asset to the team and so helpful, helping people who aren't up to speed on these things. His attitude to me was incredible, nothing was too much trouble and he wasn't rushing through things to get out.
“He's a very kind genuine man. From one very happy customer, thank you for your time.”
The future
As we move into the next phase of the partnership, we are committed to:
- expanding place-based partnerships to bring together housing, health, and social care services
- pursuing external funding to build successful initiatives
- using data and digital tools to improve service delivery and insight into the needs of our customers
- co-designing services with residents and partners to ensure they are inclusive, effective, and sustainable.
